We are currently testing the fixes for this issue plus the other one, and we hope this to be working soon. IMPORTANT: anybody who experienced issues with submission will require a new applicant record creating and resend the new link. Old links will not to work. We sincerely apologise for the inconvenience.
In addition to the earlier notification about form not submitting, it's also been reported that some fields are being populated with wrong results (e.g. pets showing "yes" when it should be "no"). We are working on this too.
